Before asking anything else, they look at it to know everything about you: your last visit, your medications, your allergies, the lab results from three months ago that you almost forgot about.

That system is the Electronic Medical Record (EMR). And while it rarely comes up in conversations about healthcare, it is, in many ways, the thing holding the entire operation together.

What is an EMR, and why does it exist?

An Electronic Medical Record is exactly what it sounds like: the digital version of a patient's paper chart. Every piece of medical history lives there — diagnoses, prescriptions, allergies, even billing information — instantly accessible to anyone in the clinic.

Before EMRs, all of this lived in physical folders. Clinicians wrote notes by hand and test results were filed on paper. The shift to digital changed that. In 2024, 95% of office-based US physicians reported using an EMR or EHR system — up from 72% using certified EHR systems in 2019 to 84% in 2024. The infrastructure of patient care has fundamentally moved online, and there's no going back.

EMR systems have massively improved healthcare operations. But one problem remains: documentation.

For every 30-minute patient visit, primary care physicians spend a median of 36 minutes on EHR tasks — before counting after-hours work. AMA's 2024 data found that of the average 57.8 hours a week physicians work, only 27.2 are spent on direct patient care. Another 13 hours go to indirect tasks like documentation, order entry, and test interpretation, and 7.3 hours to administrative work.

In short, the documentation system that was designed to enhance healthcare operations is working against it.

Where the industry is heading

The sector now recognises that the documentation burden is structural, and it's looking at a different class of solution. Rather than asking physicians to document faster, the question is becoming: what if they didn't have to document manually at all?

AI systems that generate clinical notes from clinician-patient conversations in real time are the most significant development in this space. Pilots at Mass General Brigham and Emory Healthcare showed measurable reductions in both documentation burden and burnout among clinicians who used the technology for at least 42 days. Emergency department researchers have specifically highlighted the potential of ambient documentation tools to reallocate physician time from administrative tasks back to the clinical decisions that actually require their expertise.

So the EMR isn't going away — it is, and will remain, the central infrastructure of healthcare delivery. But how information gets into it, and what that costs clinicians, is the question at the centre of healthcare technology right now.
